Eurasier Thyroid Project

Test Request Submission Form
- The only test needed for the thyroid research is the Thyroid Ab Profile (D8T) and add on TgAA to D8T, which is in red on the form, for a discounted fee of $65.00
- Please note if you want an OFA Panel done, you need to check the second test, which is Thyroid Ab Profile (D8T); add on TgAA to D8T; and OFA Panel which has a fee of $85.00, plus a separate check of $15.00 made out to the OFA
Sample Collection and Shipment Instructions
- This form contains the instructions that you will need to give to the veterinarian for the blood draw and processing for the thyroid tests. It also includes the instructions for the UK Thyroiditis Genome Project, which is optional and FREE of charge. The USEC encourages all to participate in the Genome Project
- The UK Thryoiditis Genome Project requires an additional blood draw and the completed consent form
