AKC FSS Forms

The following provides an explanation of various forms required by the AKC for different purposes, along with links to each form. Please read and adhere to the instructions on the last page of every form, as mistakes can lead to significant delays and additional fees. Use black ink and capital letters when filling out the boxes, as any missing information or writing outside the boxes may slow processing.

As with all questions on breeding, please contact the USEC Board of Directors or the Breeding Committee for questions and concerns on registering your dogs, litters and puppies. We have an on-going registration drive to help facilitate the registration of individual dogs, particularly those who are already registered with the Canadian Kennel Club.

If you are a puppy buyer, be aware that all Eurasiers in North America have historically been registered with the Canadian Kennel Club. Registration is a fairly straight forward concept, but there are some helpful Facts and Hints on this topic direct from AKC. If you are looking for a breeder and have any questions about registration, the USEC Board of Directors would be happy to assist.

Various Application Forms

Many of these forms are available through the AKC Breeder Toolkit but if you prefer to do things via snail mail, here is a helpful archive of links to various forms. 

  • Additional Signature Form – Use this form to provide additional information and signatures, when necessary (see list of forms). Mail this form with its associated form.

  • Change of Address Form – Use this form to report a change of address. The AKC® will update address records at no charge, but there are fees for issuing corrected registration documents.

  • Foundation Stock Service ® Application – Use this form to apply to record a dog in the AKC FSS® whose sire and/or dam is not recorded in the AKC FSS®.
    The following items must accompany the completed, signed application:

    • Photographs. Attach two (2) 3″ x 5″ close-up color photographs of the dog in a standing position: one front view; one side view. Print the dog’s name and registration number on the back of each photograph.

    • Pedigree. Attach an 8.5″ x 11″ photocopy of a three-generation pedigree issued by an acceptable domestic or foreign registry. For a list of acceptable registries, see the Special Registry Services pamphlet. Do not send the original pedigree. 

      • Note: If you do not have a three-generation pedigree issued by an acceptable foreign registry, you may submit export papers or any supporting documentation identifying the breed, sire, dam, and your ownership​

  • Foundation Stock Service ® Litter Application – Use this application to record a litter of puppies born in the USA out of an AKC FSS® recorded female and sired by an AKC FSS® recorded male of the same breed.​

  • Request for FSS® Duplicate Recording Certificate – Use this form to request a duplicate AKC FSS® Recording Certificate for a dog. This request must be completed and signed by the owner of the dog as shown on AKC FSS® records. When a duplicate certificate is issued, the original certificate becomes invalid.

  • Foundation Stock Service® Duplicate Litter Kit Request – Use this form to request a duplicate AKC FSS® litter kit. This request must be completed and signed by the owner of the litter as shown on AKC FSS® records. To request an individual application instead of an entire litter kit, please use the Request for a Duplicate AKC FSS® Dog Recording Application form. 

  • Foundation Stock Service® Litter Correction Form – Use this form to correct information that was inaccurately reported on the original AKC Foundation Stock Service® Litter Application you submitted. 

  • Foundation Stock Service® Special Litter Application – Use this application to record a litter of puppies whelped in the USA from an AKC FSS® recorded female imported in whelp or from an AKC FSS® recorded dog mated to an AKC FSS® dog enrolled for breeding purposes only. 

  • Foundation Stock Service® Multiple Sire Litter Registration Application – Use this application to record litters of puppies with more than one sire. The litter must be bred and whelped in the USA, and the puppies being recorded must be from an AKC FSS® recorded female and an AKC FSS® recorded male of the same breed.

  • Foundation Stock Service® Dog Name Change Authorization – This application must be completed to effect a change to an AKC FSS recorded dog’s name. The name of an AKC FSS recorded dog may be changed provided the dog was whelped in the United States.

  • AKC DNA - Determines the unique genetic markers for an individual dog. Required for all first generation (imported or American-born dogs that were originally registered with the Canadian Kennel Club) as well as Frequently Used Sires (dogs having the greatest impact on the AKC Stud Book); dogs whose semen is collected for fresh-extended or frozen use; and for the sires, dam and puppies for Multiple-Sire Litter Registration.

    • AKC DNA + Health - This test establishes a dog's AKC DNA Profile, which is unique to each individual dog, as well as testing for over 328 markers for health and phenotypical traits.​ This is more expensive because it includes testing for carrier status for the markers for health, coat color,  coat length, etc. It is not necessary but by combining AKC DNA + Health it is less expensive than running the two tests separately.
